Use this form to exercise a right the law gives you with Assurance Boréale: access (P-39.1 s. 27), rectification (s. 28), ceasing dissemination or de-indexing (s. 28.1), access in a structured format, or information about an automated decision (s. 12.1). Assurance Boréale will receive your request and, if it can act on it, must respond in writing within 30 days (s. 32). The company may first ask you to prove your identity before acting on your request (s. 30). Agreely records your request and tracks that deadline; Assurance Boréale verifies your identity and decides the outcome. Agreely does not adjudicate the request.
